PURPOSE:

• Allow recognition of any German Shepherd Dog – no special title or award necessary – your trusted friend and lifelong companion
• Raise funds in support of research of diseases affecting the German Shepherd Dog

GUIDELINES:

• Must be a German Shepherd
• No special title or award necessary
• Donation $150.00
• Dog and owner’s name to be engraved on sculpture
• Mail funds payable to the AMERICAN GERMAN SHEPHERD DOG CHARITABLE FOUNDATION, INC. to F. Susan Godek, 11059 East Sunnyside Drive, Scottsdale, AZ 85259-3074
• Print or type name of dog and owner – enclose a picture and brief write-up – a catalog of all dogs recognized will be printed
• Names may be submitted at anytime – however must be received three weeks prior to the National to be added for current year
